Registration Flow

Watch or attend the Good Hygiene Practices (GHP) seminar for drivers and porters.

Step 01

Hand over the initial documents to the Meat Inspection Officer (MIO). Bring your delivery van for inspection at the meat establishment.

Step 02

Once inspection is done, submit all complete documentary requirements to the NMIS Regional Technical Operations Center (RTOC).

Step 03

NMIS will issue your Order of Payment after reviewing your documents.

Step 04

Pay the corresponding MTV registration fee.

Step 05

After payment, receive the MTV Certificate of Registration and official MTV sticker to display on your vehicle.

Step 06

Important Notes

Registration is annual. Apply at least two (2) months before expiration date.
Always carry updated permits and sticker.
Expired or unregistered MTV units are not allowed to load or unload meat products.

List of Requirements

Duly Accomplished and Notarized MTV Application Form
Application form completed and notarized as prescribed by NMIS regulations.
Valid Proof of Company Registration
Complete copy of the DTI Certificate of Business Name Registration for sole proprietorships, or SEC registration documents for partnerships/corporations, where applicable.
Valid Certificate of Registration (COR)
LTO Certificate of Registration under the name of the owner, applicant, or company.
Temporary or Improvised Plate Authorization
LTO certificate authorizing use of a temporary or improvised plate number, for vehicles using a temporary or unofficial plate number, where applicable.
Updated Official Receipt (OR)
Updated LTO vehicle registration official receipt.
GHP Certificate of Completion or Attendance
Certificate for the driver and helper (pahinante) on Good Hygienic Practices in meat/food handling conducted by the National Meat Inspection Service.
Valid Health Certificate(s)
Health certificates for the driver and helper issued by the City or Municipal Health Office.
Clear Colored Vehicle Photos
Clear colored 7 × 5 inch (or 5R) photos: front view showing plate number, left and right views, closed-back view showing plate number, and open-back view showing the MTV compartment.
Signed Certification or Contract from the Licensed Meat Establishment
Certification or contract indicating that the MTV will be engaged to render service for the licensed meat establishment.
